- Provide safe Physical Therapy interventions to clients/families in accordance with agency policies and procedures.
- Conduct client assessments using best clinical practice.
- Develop appropriate plans of care.
- Maintain client care records using approved documentation and format.
- Documentation reflects care and interactions.
- Documentation is completed in a timely manner.
- OASIS documentation is completed within regulatory timeframes.
- Documentation is clear and concise.
- Currently registered by New York State and the American Physical Therapy Association and educational standard board as applicable.
- Current New York State Physical Therapy license.
- Current New York State driver's license and automobile insurance.
- One-year experience as a therapist preferred.
The Physical Therapist will be required to sit or stand at a desk 90% of the day. They will also be required to perform tasks involving physical activity, including pushing/pulling on average 50 lbs with a max of 150 lbs, lifting/carrying on average 25 lbs with a max of 40 lbs, patient transfers, extensive bending, standing, kneeling, walking in various weather conditions, and ability to go up and down stairs.
