Job Summary

C.O.R.E. Medical Clinic, Inc. is a CARF-Accredited Healthcare Facility seeking a Licensed Nurse Practitioner or Physician Assistant to fill the position of part-time Allied Health Professional. The ideal candidate will provide coverage at our Midtown Sacramento and West Sacramento locations, working 2-3 days a week with a minimum of 20 hours per week. Job Responsibilities
  1. Identify patients' health status by taking medical histories, performing physical examinations, and evaluating patients for program eligibility.
  2. Plan and implement a course of medical treatment, including ordering medications within standardized procedures.
  3. Counsel and instruct patients and families regarding health-related matters.
  4. Assist in determining which cases to refer to a Physician for further diagnosis.
  5. Provide and/or coordinate healthcare services by referral, with follow-up for concurrent medical problems not related to addiction.
  6. Under medical direction or within established protocols, order and interpret laboratory tests.
  7. Maintain records in our Electronic Health Record and prepare reports as needed.
  8. Meet and educate pregnant patients, prescribe buprenorphine, and provide medication management.
  9. Perform related work as required, including completing annual physical examinations on patients.
