Job Summary
We're seeking a highly motivated Community Health Advocate to join our team. In this role, you'll assess, plan, and implement care strategies tailored to individuals' unique needs.
You'll identify and initiate referrals for social service programs, manage person-centered service plans, and communicate with stakeholders to ensure quality coordinated care.
Your advocacy will help ensure individuals' needs and choices are fully represented and supported by the healthcare team.
Responsibilities
- Assess and develop individualized care strategies
- Identify and initiate referrals for social service programs
- Manage person-centered service plans throughout the continuum of care
- Communicate with stakeholders to ensure quality coordinated care
- Advocate for individuals and families as needed
Requirements
- Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area
- 2+ years of experience working within the community health setting in a healthcare role
- 1+ year of experience working with individuals with long-term care needs
- 1+ year of experience working with individuals receiving services on home and community-based waivers
- Ability to travel to visit Medicaid members in their homes and other settings
Estimated Salary: $65,000 - $80,000 per year
