Key Responsibilities- Conduct comprehensive assessments of patients to determine their care needs and develop personalized plans of care.
- Coordinate with other healthcare professionals to ensure seamless care delivery and achieve continuity of patient care.
- Provide and direct provisions of nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Make initial and comprehensive nursing evaluation visits, determine primary focus of care, and develop plans of care within state-specific guidelines.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
