About the Job:

Fusion Medical Staffing-Therapy is seeking a skilled Physical Therapist to join our team for a travel assignment in a Home Health setting. As a Physical Therapist with Fusion Medical Staffing-Therapy, you will have the opportunity to work with a variety of patients and provide high-quality care in a dynamic and supportive environment.

Job Summary:

We are looking for a Physical Therapist to work in a Home Health setting, providing physical therapy services to patients in their homes. The ideal candidate will have experience working in Home Health and be able to provide high-quality care to patients with a variety of conditions.

Responsibilities:

  • Provide physical therapy services to patients in their homes
  • Develop and implement individualized treatment plans to meet the needs of each patient
  • Conduct thorough assessments of patients to identify their physical therapy needs
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care
  • Document patient progress and treatment plans in a timely and accurate manner

Requirements:

  • Current licensure as a Physical Therapist in the state of California
  • Experience working in Home Health or a similar setting
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to lift, push, and pull equipment and supplies as needed
