About the Role

Fairview Health Services is seeking a skilled Medication Documentation Specialist to join our team at the University of Minnesota Medical Center West Bank campus.

Key Responsibilities
  • Prepare accurate and up-to-date patient-specific medication lists using information from patients, families, pharmacies, and other care facility records.
  • Identify and clarify potential drug-related issues in the medication regimen.
  • Efficiently triage patients to ensure an accurate medication list is available for physician review.
  • Enter medications and allergies into patient profiles as required by protocols.
  • Perform other administrative duties as assigned.
Requirements
  • 1-2 years of healthcare experience and/or equivalent educational experience.
  • Preferred qualifications include vocational/technical training or an associate degree, 2 years of experience as an LPN, CMA, CNA, TMA, or in a pharmacy environment, and at least 6 months of experience conducting medication histories or similar patient-focused communication.
  • License/Certification/Registration: LPN, CMA, CNA, TMA, or Certified Pharmacy Technician.
