**Job Responsibilities:** As a Patient Care Coordinator, you will be responsible for:
- Delivering personal care services to patients in various healthcare settings.
- Taking vital signs and documenting patient information according to facility protocols.
- Communicating effectively with patients, families, and healthcare teams.
**Requirements:**
- Completion of a state-approved certified nursing program and/or successful passing of a competency evaluation program which meets state regulations.
- Meets state specific certification requirements and possesses unrestricted certification as a CNA.
- At least six months of work experience as a CNA in a healthcare setting.
