We are seeking a skilled RN Case Manager to join our team at Northwestern Memorial Healthcare. As a RN Case Manager, you will play a critical role in ensuring the efficient and effective management of patient care, working closely with the multidisciplinary team to coordinate care and services.
Key Responsibilities- Assess patients and determine case management needs, intervening as appropriate to ensure timely and appropriate care coordination.
- Participate in the development and implementation of patient care plans, working collaboratively with the multidisciplinary team to ensure optimal quality and efficiency of patient care and services.
- Collaborate with patients, families, and caregivers to facilitate prioritization of care along the continuum, ensuring that patients receive the necessary resources and support to achieve optimal outcomes.
- Communicate effectively with healthcare teams, physicians, and other clinical providers to manage and direct care, reducing variances from expected outcomes and ensuring compliance with regulatory and accreditation standards.
- Document assessments, actions, and patient outcomes, ensuring accurate and timely documentation of patient care and services.
- Graduate of an accredited school of professional nursing.
- Three or more years of experience in acute care nursing, including inpatient and outpatient settings.
- Licensed to practice as a Registered Nurse in the state of Illinois.
- Bachelor of Science in Nursing.
- Case Management, Utilization Management, Discharge Planning, or related experience.
