Salary
The estimated salary range for this role in Stockton, CA is $207,194 - $290,339 per year, with additional incentives up to $25,445. This competitive total rewards package includes a 401(k) match, healthcare coverage, and a broad range of other benefits.
Job Description
As a Nurse Practitioner at WelbeHealth, you will work collaboratively with our interdisciplinary team (IDT) to evaluate and guide participant care. Your key responsibilities will include:

- Performing in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments, as well as continuous medical coverage to participants;
- Collaborating with physicians and IDT to develop care plans for each participant;
- Collaborating with physicians and IDT in discharge planning from acute or long-term settings;
- Helping maintain care of participants in the nursing home, making regular visits as dictated by participant need;
- Working collaboratively with WelbeHealth physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination;
- At the direction of the Medical Director, reviewing outcomes and utilization data, including hospitalizations, and using specialists to establish best practices and policy improvements;
- Taking part in rotating night and weekend call.
Required Skills and Qualifications
To be successful in this role, you will need:

- Graduation from an Accredited School of Nursing;
- Certificate as a Nurse Practitioner with Prescriptive Authority;
- RN State License;
- Active Drug Enforcement Administration (DEA) registration;
- Current enrollment or eligibility for enrollment to provide services to Medicare and Medi-Cal participants;
- Minimum of two years of experience working with the senior population;
- Current knowledge of chronic care/geriatric issues and best practices;
- Reliable means of transportation.
