Responsibilities- Assist the MDS Coordinator with the Minimum Data Set (MDS) process, ensuring compliance with state and federal guidelines.
- Conduct thorough assessments of residents' physical and mental functions, documenting data accurately and efficiently.
- Determine referrals to other healthcare professionals and implement care plans using the Care Area Assessments.
- Organize and manage the MDS process for all residents, ensuring timely completion and accuracy.
- Attend daily morning meetings to monitor order changes, significant changes, and other important updates.
- Participate in daily PPS meetings to manage Medicare, Managed Care, and Medicaid processes.
- Lead or assist in leading resident care plan meetings, ensuring effective communication and follow-up on issues.
- Maintain the MDS schedule and communicate with the Interdisciplinary Team (IDT) for PPS and OBRA MDS.
- Complete annual, quarterly, and significant change/discharge MDS assessments.
- Assist the Director of Nursing (DON) in monitoring the review of nurse's notes to ensure care plans are being followed.
- Attend scheduled in-services, staff meetings, workshops, and seminars to stay up-to-date on best practices.
- Comply with safety requirements, infection control measures, and maintain a clean work environment in accordance with facility policy.
- Graduate of an accredited school of nursing with a valid RN or LPN license in good standing according to State and Federal requirements.
- Understanding of the philosophy and principles of the MDS process.
- Thorough knowledge of safe, effective nursing practices.
- Managerial and interpersonal skills.
- Patience, a cheerful disposition, enthusiasm, and willingness to handle residents with varying maturity and physical levels.
