Key Responsibilities:- Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
- Conduct thorough assessments, develop individualized treatment plans, and implement evidence-based interventions to promote patient progress.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordinate services with other healthcare professionals.
- Document patient progress, maintain accurate records, and communicate effectively with patients, families, and healthcare teams.
- Participate in ongoing education and training to stay current with best practices and industry developments.
