Primary Care Nurse Practitioner

CINQCARE is seeking a highly skilled Primary Care Nurse Practitioner to join our team of healthcare professionals. As a key member of our primary care team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes, assisted living facilities, and independent living communities.
Key Responsibilities:
- Conduct comprehensive in-home assessments to identify patient needs and develop personalized care plans
- Review patient symptoms and medical history to inform diagnosis and treatment decisions
- Perform physical examinations, medication reviews, vital checks, lab draws, wound care, and depression screenings
- Communicate assessment findings to primary care physicians and other healthcare team members to ensure seamless care coordination
- Meet quality metrics and closure rates while maintaining high patient satisfaction and improved health outcomes
- Travel frequently to provide care to patients in their homes, often in various weather conditions
Requirements:
- Education: FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA
- Unrestricted NP or PA license in the state of IN
- Nurse Practitioner certification from either the ANCC / AANP
- Current DEA licensure / Prescriptive Authority post-hire
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- 2+ years of clinical experience as an NP/PA in primary care or home care setting
