Position Overview
The Practical Nursing Specialist collaborates with an interdisciplinary team and operates under the guidance of licensed professionals, such as Registered Nurses or Physicians. This role is essential in delivering skilled nursing care and administering medications to patients.

Key Responsibilities
1. Provide empathetic care to patients and their families.
2. Educate patients and their families on health maintenance and preventive measures.
3. Offer emotional support and companionship, creating a nurturing environment.
4. Assist in organizing healthcare services, including appointment scheduling and transportation arrangements.

Patient Interaction
Engage with patients, family members, and other clients with empathy and respect for confidentiality. Communicate courteously and maintain a professional demeanor.

Nursing Process Participation
Under the supervision of licensed professionals, actively participate in the nursing process, which includes planning, implementing, and evaluating patient care plans in a timely and accurate manner, while collaborating with other healthcare team members.

Direct Patient Care
Deliver direct nursing care and assist with patient management as directed by a Registered Nurse or other licensed professionals.

Data Collection and Documentation
Conduct thorough patient assessments, considering various factors such as age, condition severity, and individual needs. Collect and document both objective and subjective data, ensuring adherence to established charting guidelines.

Medication Administration
Administer prescribed medications and treatments within the scope of practice, utilizing appropriate medical equipment as necessary.

Evaluation of Care
Contribute to assessing patient responses to nursing interventions, involving patients and their families in the evaluation process. Document and communicate findings to relevant healthcare team members.

Qualifications
- Current Licensed Practical Nurse (LPN) certification from the state of employment or LPN applicant status (required).
- Basic Life Support (BLS) certification from the American Heart Association (required).
- Additional certifications such as ACLS, PALS, or NRP may be required for specialized areas.

Experience and Skills
- Preferred: 1 year of clinical patient care experience in settings such as hospice, acute care, or long-term care.
- Familiarity with Electronic Health Record (EHR) systems is preferred.

Patient Population Knowledge
Demonstrate the ability to provide care tailored to the age and specific needs of patients across various life stages, from infants to geriatrics.

Physical Requirements
Ability to perform physical tasks such as lifting, carrying, and standing for extended periods, with manual dexterity and the ability to respond to auditory signals.
