Position Overview
As a vital member of the healthcare team, you will provide clinical leadership for patient care in an acute inpatient environment, emphasizing patient and family-centered approaches. Your clinical expertise will be demonstrated through the application of evidence-based practices across all nursing processes: assessment, planning, implementation, evaluation, and outcomes. You will integrate knowledge of safety protocols, cost management, and quality enhancement throughout your nursing duties.

Compliance with all organizational policies and procedures is essential, including but not limited to: licensure renewal, mandatory training, employee health evaluations, attendance policies, dress code adherence, patient confidentiality, and medication administration.

You will be expected to have a thorough understanding of regulatory agency standards (TJC, SC DPH, CMS) and provide care in accordance with the South Carolina Nurse Practice Act.

This role may involve scheduling for weekends, holidays, and on-call duties.

Key Responsibilities

Assessment:

Conduct comprehensive nursing assessments using appropriate standards and tools specific to the patient's age and condition. Document and recognize changes in patient status and responses to interventions. Establish nursing priorities for the care plan based on assessments and collaborate with the healthcare team for validation.

Planning/Outcomes:

Create individualized care plans that reflect the patient's age and needs, developed in partnership with the patient, family, and healthcare team. Regularly update the plan based on the patient's evolving condition and communicate these changes effectively.
Set realistic and measurable goals for discharge, ensuring they are validated with the patient and family.

Respect the cultural, spiritual, and personal beliefs of patients and families during care planning.


Include discharge planning to ensure continuity of care and a seamless transition post-discharge, including necessary referrals.

Implementation:

Deliver direct care to assigned patients in alignment with the established nursing plan.

Execute specific tasks in accordance with organizational policies and within the RN scope of practice.


Delegate nursing responsibilities appropriately to healthcare team members, ensuring tasks are completed effectively.

Continuously monitor the quality of care provided, ensuring appropriate oversight of assigned patients.


Prioritize patient care based on unit demands, coordinating tasks efficiently and utilizing available resources.

Incorporate cost-effective strategies into nursing care while leveraging technology in daily practice.


Ensure patient safety by maintaining a secure environment and adhering to established care standards.

Utilize performance metrics to gather data and assess the quality and effectiveness of nursing care.

Participate in hospital/unit performance improvement initiatives.

Evaluation:


Evaluate the effectiveness of the care plan based on patient responses to interventions, documenting progress towards expected outcomes and adjusting the plan as necessary.

Report any changes in patient condition and refer unresolved issues to the appropriate healthcare team members. Engage in discussions about outcomes with peers and other team members.

Identify opportunities for quality improvement and integrate performance enhancement recommendations into the care plan as needed.

Patient and Family Education:

Assess the educational needs of patients and families, providing instruction that is clear and relatable by:

- Using familiar language and terminology

- Offering examples to clarify concepts

- Implementing and assessing a teaching plan that encourages patient and family involvement

  • Utilizing computer resources and Micromedex effectively
Communicate the educational needs of patients and families to the broader healthcare team.

Professional Accountability:


Reflect on your nursing practice in relation to professional standards, state regulations, and current knowledge.

Take responsibility for maintaining competency through ongoing education and professional development. Ensure timely completion of mandatory education and maintain professional licensure. Identify strengths and areas for improvement in your practice.

Ensure that documentation and communication focus on relevant information to meet patient and family care needs.

Act as an advocate for patients, coordinating care collaboratively with providers and other healthcare professionals.

Supervisory/Management Responsibilities

Minimum Qualifications

Associate's Degree in Nursing; BSN preferred.

One year of experience as a registered nurse.

Licensure/Certifications Required

Must hold a valid Registered Nurse license in South Carolina.

Alternative Qualifications


In lieu of one year of registered nurse experience, successful completion of the Prisma Health Nurse Residency Program or a comparable residency from another organization will be accepted.

One year of licensed practical nurse (LPN) experience may also be considered.
