About the Role:
The Hospice Admission Nurse will be an expert in the admission process and be well-versed in hospice eligibility. They will assist with hospice talks, providing information about the hospice benefit for potential hospice patients and their families.
Key Responsibilities:
- Provide skilled nursing services to patients in their own homes under the direction of a physician's plan of care.
- Develop plans of care with realistic and measurable outcomes in collaboration with the patient/family.
- Provide education and promote patients' rights and responsibilities for informed decision-making in the plan of care.
- Assess and identify patients' holistic healthcare needs.
- Demonstrate competency in assessment and technical skills.
- Possess and utilize knowledge of pain control and symptom management.
Requirements:
- Current R.N. license in the State of Maine.
- ANA certification in Home Health or Community Health Nursing encouraged.
- Current CPR card required.
- One year of recent clinical nursing experience required.
- A reliable car with insurance.
