Job Summary

The Home Health Nurse (LPN) will provide nursing care to clients in their place of residence, under the supervision of a registered nurse. The nurse will coordinate care with the Home Health LPN interdisciplinary team, clients, and their families, and the referring Agency.

Key Responsibilities
  • Assist in identifying the client's physical, psychosocial, and environmental needs
  • Participate in planning and implementing care in conjunction with the RN, in accordance with the Plan of Care (POC)
  • Assure the continuity of care through the delivery of quality patient care
  • Provide effective communication to clients, their families, team members, and other health care professionals
  • Monitor assigned cases to ensure compliance with the requirements of third-party payers
Requirements
  • 1-year experience as an LVN/LPN in a clinical care setting
  • Home Health experience
  • Nursing skills as defined as generally accepted standards of practice
  • Excellent interpersonal skills and ability to communicate effectively
  • Demonstrates proven decision-making skills
