We are seeking a skilled Registered Nurse to join our Home Health team at Altru Health. As a RN Home Health, you will provide high-quality patient care and lead a team of healthcare professionals to deliver exceptional patient outcomes.
Key Responsibilities- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Provide education and support to patients and their families to promote health and wellness.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Supervise and direct the work of LPNs and aides to ensure high-quality patient care.
- Current RN licensure in North Dakota or Compact State Agreement.
- Basic Life Support (BLS) certification within 90 days of hire.
- Minimum 1 year of related experience in home health or a similar setting.
This role requires frequent driving and working in a fast-paced environment. The ideal candidate will possess excellent communication and leadership skills, with a passion for delivering high-quality patient care.
