We are seeking a skilled Licensed Practical Nurse to join our team at Legend DDD Services. As a key member of our nursing staff, you will be responsible for providing high-quality patient care and support to our members in our nursing-supported group homes.
Key Responsibilities- Provide safe and effective care to members, including administering medications, performing routine nursing duties, and assisting with daily living activities.
- Collaborate with the Chief Nursing Officer and NSGH Administrator to ensure that members receive comprehensive care and support.
- Document all medical aspects of care and changes in conditions regarding members in the home.
- Perform routine nursing duties, including taking vital signs, providing comfort and hygiene care, and managing medications.
- Assist with showering members, changing/repositioning them, cooking meals, and assisting with cleaning the home.
- Ensure that all required documentation is completed before the end of shift.
- Current Arizona State Licensed Registered Nurse or Practical Nurse license (in good standing).
- Maintain standards of care as outlined in the Nurse Practice Act by Arizona State Board of Nursing.
- Flexible hours to work times as well as work locations to assist with staff coverage if needed.
- Reliable transportation to work at any location.
- Must have or be capable of passing and maintaining Article 9 and Prevention and Support within 90 days of hire.
- Must have or be capable of passing and maintaining CPR within 30 days of hire.
- Must have or be capable of obtaining and maintaining a Class 1 Fingerprint Clearance Card.
- Ability to work as a team member.
- Adaptability & Flexibility to work times as well as work locations to assist with staff coverage if needed.
- Ability to positively interact with members, their families, and administrative staff.
- Recognize and be responsive to the needs of the members.
- Ability to maintain proprietary and resident information confidentially.
- Ability to maintain a calm, non-defensive, supportive attitude during crisis or potential crisis.
- Proficient verbal and written communication in English and ability to do so with tact.
