We are seeking a compassionate and caring Registered Nurse to join our team at Homeland Center. As a Registered Nurse, you will provide skilled professional nursing care to home health patients, as prescribed by the physician and in compliance with the Home Health Care Medicare Conditions of Participation, Pennsylvania Home Health Care licensure laws, and agency policies and procedures.
Responsibilities:- Provide initial and comprehensive nursing assessment of patient needs and collaborate to develop the home health plan of care.
- Reevaluate and revise the plan of care relative to the patient's nursing needs to provide appropriate, comprehensive and responsive goals and interventions.
- Work closely with other members of the health care delivery team to ensure all patient and family care needs are being met.
- Supervise Licensed Practical Nurses (LPN) and Home Health Aides.
- Graduate of an accredited registered nursing program.
- Bachelor's degree in Nursing preferred.
- Minimum of one (1) year general nursing experience, preferably with current home health, medical, surgical or critical care experience.
- Ability to work within an interdisciplinary setting.
- Ability to work independently.
