Inpatient RN Care Manager

The Inpatient Nurse Care Manager will collaborate with the multidisciplinary team to assess, plan, implement, and evaluate patients, incorporating clinical, psychosocial, and family factors specific to the care and disease management plan.
Key Responsibilities- Communicate and manage transitions of care, assisting patients and families in navigating the healthcare system.
- Recognize and intervene to secure resources necessary for patient and family care and safety across the continuum of care.
- Collaborate with financial payers/insurers to secure resources for patient and family care and safety.
- Provide assessment and evaluation of patients for clinical, psychological, and social factors contributing to their disease process and continuing care needs.
- Interface with patients, physicians, nurses, social workers, and other hospital departments to affect changes in case management and secure necessary resources.
- Provide information to third-party payers with required documentation of medical necessity.
- Support patients and families, providing evaluation and coordination of care planning and patient safety while arranging for transitional care and discharge planning needs.
- Determine if patients are meeting criteria for evidence-based clinical guidelines for appropriate level of care through utilization review.
- Identify the need to execute letters when medical necessity is not met and issue letters of patient liability regarding hospital admission.
- Associates in Nursing degree from an accredited nursing program; Bachelor's degree in Nursing preferred.
- Current NH RN License.
- Acute Care Facility experience of 2 years in a clinical specialty or Care Management, Discharge planning, Quality Improvement, and Utilization Review.
- Effective interpersonal, organizational, and motivational skills.
- Ability to function as a team player.
- Proficient in written and verbal communication.
