Position Overview

Role: Licensed Practical Nurse (LPN) specializing in hospice care.

Responsibilities:

  • Implement the care plan established by the supervising Registered Nurse.
  • Educate patients and their families on health management and preventive care strategies.
  • Encourage patient and family autonomy in managing their health needs.
  • Deliver compassionate physical, emotional, and spiritual support to patients.
  • Assess home safety and recommend necessary modifications to ensure a secure environment.
  • Apply the nursing process to execute the care plan effectively for patients and their families.
  • Provide immediate care in emergency situations as required.
  • Engage in community health assessments and participate in health education initiatives.
  • Maintain personal health and hygiene, ensuring physical and mental well-being.

Qualifications:

  • Graduate from an accredited Practical Nursing program with completion of pharmacology coursework.
  • 2 to 3 years of recent experience in acute care nursing or a related field; experience in community health nursing is a plus.
  • Valid LPN license in the State of Maine and current CPR certification.
  • Possession of reliable transportation, valid insurance, and a Maine Driver's license.
