Key Responsibilities:

  1. Deliver evidence-based services while maintaining fidelity to promote continuity of care for clients. Provide services in their residences or other community settings as needed.
  2. Arrange for medication evaluation, management, and monitoring to ensure optimal client care.
  3. Seek and utilize clinical supervision to provide high-quality mental health services.
  4. Maintain an active caseload as directed by the supervisor, meeting Communicare and Department of Mental Health standards.
  5. Meet all Communicare, Department of Mental Health, and Division of Medicaid standards and policies regarding patient care and documentation.
  6. Provide timely services and documentation as directed, meeting productivity requirements.
  7. Efficiently navigate Communicare's Electronic Health Record system.
  8. Attend meetings and training as assigned, successfully completing crisis intervention and suicide prevention training.
  9. Assist in collecting and maintaining data as needed or required.
  10. Participate in the Communicare treatment team.
  11. Provide rapid response crisis services, intakes, and therapy as directed by the supervisor.
  12. Assess home and family situations related to client needs, working directly with families to ensure their participation in treatment.
  13. Meet with referral sources and other outside agencies pertinent to the individual treatment plan.
  14. Collaborate with other Communicare programs to maintain continuity of care and a coordinated system of care.
  15. Establish and maintain referral bases within the community.
Requirements:

Qualifications:

  1. A Master's Degree and license or certification in counseling, psychology, social work, marriage and family therapy, or other related behavioral health care fields are preferred. If not already licensed or certified, the candidate must be eligible for licensure or Department of Mental Health certification within thirty days.
  2. The candidate must pass a Communicare background check and drug screen, as Communicare is a drug-free workplace.
  3. The candidate must have the ability to perform the responsibilities of the position as described above.
  4. The candidate must be able to work independently.
  5. The candidate must be able to relate positively to individuals referred to Communicare, their families, other staff members, community members, and staff from other agencies.
  6. The candidate must have reliable transportation and the ability to travel throughout the catchment area and to trainings and meetings in other locations as needed. A valid driver's license, automobile insurance, and an insurable driving record must be maintained continuously throughout employment.

Salary details: $35,000 - $37,000 per year
