Substance Abuse Counselor

Catholic Health Service is seeking a highly skilled Substance Abuse Counselor to join our team. As a Chemical Dependency Specialist, you will provide chemical dependency counseling, intervention, and discharge planning to adult individuals with chemical dependency and/or mental health problems.

Key Responsibilities:

• Provides chemical dependency counseling, intervention, and discharge planning as a member of an interdisciplinary team.
• Screens potential individuals for admission via telephone/walk-ins to ensure appropriate level of care.
• Refers individuals to other referral services if the individual is not appropriate for the program's level of care or if there is no bed availability.
• Collects concise, accurate individual Bio-Psychosocial information and documents the admitting note.
• Develops and documents an individualized Recovery Plan within 24 hours of admission.
• Provides assistance to individuals with aftercare linkage, individual crisis counseling, and basic education about the disease process.
• Develops educational topics and facilitates group counseling.
• Plans effective and timely discharges based on utilization review criteria.

Requirements:

• CASAC, LMHC, or LMSW with experience in substance abuse required.
• High School diploma required. Bachelor's degree in Human Service field preferred.
• One (1) year working with individuals with chemical dependence and/or psychiatric experience preferred.
