About the Role

We are seeking a highly organized and detail-oriented Care Coordinator Assistant to join our team at CommonSpirit Health. As a key member of our Care Coordination department, you will provide administrative support to our licensed staff, ensuring the smooth operation of our office and the delivery of exceptional patient care.

Responsibilities
  • Provide clerical and secretarial support to the Care Coordination team, including data entry, filing, and record-keeping.
  • Assist with post-acute care coordination, revenue-related activities, and stocking and monitoring office supplies and equipment.
  • Serve as the point of contact for the department, responding to calls, messages, and forwarding communications to the responsible party.
  • Perform duties such as scheduling, faxing, auditing, transcribing, filing, delivering, sorting, editing, tracking, and correlating department documentation records and forms.
  • Optimize licensed staff productivity by providing clerical support, managing confidential patient information, and ensuring compliant and accurate management of departmental records and communications.
Requirements
  • Associate Degree or High School Graduate with a Medical Office Certificate from an accredited institution.
  • One (1) year experience supporting clinicians in demanding and fast-paced patient care environments and/or medical office experience.
  • Proficiency in Microsoft Office applications, including Word, basic Excel spreadsheet manipulation, typing, and data entry skills.
  • Excellent customer service and presentation skills.
  • Strong interpersonal and written communication skills.
  • Demonstrated ability to apply analytical and problem-solving skills.
  • Demonstrated ability to manage multiple tasks or projects effectively.
  • Ability to work independently as needed with a high degree of detail orientation.
  • Ability to work efficiently in a fast-paced environment with changing priorities.
  • LVN License Preferred.
