At Physicians Dialysis, we are seeking a highly skilled and compassionate Primary Therapist to join our team. As a Primary Therapist, you will play a vital role in helping patients recover from substance abuse and co-occurring disorders through mental/addiction counseling, education, treatment, and empathetic support.
Key Responsibilities:- Develop and implement individualized treatment plans for clients that include measurable and time-limited goals and objectives.
- Conduct follow-up therapy sessions to monitor progress and adjust treatment plans as needed.
- Complete assessments and re-assessments in a timely manner to ensure accurate and effective treatment.
- Act as a mental health liaison between nursing and operations staff to ensure seamless communication and collaboration.
- Provide one-on-one intervention and follow-up for clients with substance abuse and/or mental health complications.
- Monitor and document treatment responses and progress reports for clients.
- Ensure complete, accurate, and timely entries are made for each client contact.
- Perform biopsychosocial assessments, evaluate mental health status, and perform follow-ups in a timely manner.
- Monitor that standards of care are in accordance with NCCHC/ACA or accepted psychological standards.
- Develop and conduct in-service education programs according to annual in-service calendar.
- Prepare monthly reports that reflect performance and submit copies to the designated Supervisor.
- Actively assist with the detection and prevention of client suicide.
- Actively communicate with the Clinical Director and Utilization Review regarding client status and progress.
Required Qualifications:
- Relevant license and/or certification as a Mental Health provider in the State of Kansas (i.e., Psychologist, Licensed Professional Counselor-LPC, Licensed Clinical Social Worker-LCSW, Licensed Marriage and Family Therapist-LMFT).
- BLS Provider obtained within 1 month (30 days) of hire date.
Preferred Qualifications:
- Minimum of two years' experience working in a mental health field.
- Previous substance abuse treatment and/or detox facility experience.
- Flexibility in work arrangements and ability to work alone when taking calls or covering evening and/or night shifts.
- Strong communication skills amongst clients and within all necessary departments.
- Able to utilize an electronic medical record.
