Clinical Social Worker

We are seeking a highly skilled Clinical Social Worker to join our team at Lowell General Hospital. As a key member of our healthcare team, you will provide a range of clinical social work services to patients and families in inpatient, ambulatory, and community settings.
Key Responsibilities- Conduct psychosocial assessments and develop treatment plans with appropriate interventions.
- Provide crisis intervention and support in situations involving suicide, homicide, family violence, elder or child abuse.
- Collaborate with the healthcare team to promote a smooth, coordinated plan of care and identify psychosocial issues that may impede progress.
- Provide medical social services to patients' families or caregivers on a short-term basis when necessary.
- Participate in multi-discipline case conferences and consult with agency care providers regarding social service needs of clients and families.
- Master's degree in Social Work from an accredited institution.
- Licensed Clinical Social Worker (LCSW) licensure in NH may be required depending on assigned territory and operational need.
- One year of related experience.
