The Registered Nurse (RN) plays a vital role in delivering a restorative healing environment for patients and their support systems, working in partnership with the Clinical Manager and the interdisciplinary team. As a dedicated professional, the RN exercises autonomy, authority, and control to ensure outstanding patient outcomes. Professional accountability is demonstrated in five key areas: practice, quality assurance, competence, research, and resource management.
Key Responsibilities:
- Patient Assessment: The RN exhibits independence in practice by conducting comprehensive and ongoing holistic nursing assessments, including essential admission evaluations of patients and their support systems. Upon admission, the RN identifies, prioritizes, and documents current and potential issues to develop a tailored care plan that addresses specific needs and concerns. The RN formulates appropriate interventions and desired outcomes for critical issues to achieve positive patient results, continually evaluating based on patient condition and established policies.
- Patient Care and Discharge Planning: Nurses are responsible for their practice in coordinating care. The RN collaborates with patients, their support systems, and relevant healthcare team members to address immediate needs, discharge requirements, and outcomes for significant issues. A care plan is implemented upon admission or within 24 hours. Recognizing the importance of interdisciplinary decision-making, the RN initiates necessary referrals and regularly evaluates, revises, and documents the patient’s care plan.
- Patient Education: The RN is empowered to utilize independent judgment and critical thinking to provide individualized, ongoing education to patients and their support systems throughout the care continuum. Teaching is an essential aspect of nursing practice, and the RN employs current, evidence-based methods in their educational efforts.
- Effective Communication: As a professional, the RN fosters mutual respect and collaboration in communications with the interdisciplinary healthcare team. Communication is a reciprocal process where the RN utilizes effective skills such as active listening, relationship building, and assertive techniques. The RN advocates for patient rights and strives to cultivate high-performance teams by promoting a supportive and inclusive environment.
- Treatment and Procedures: As a key provider of healthcare, the RN performs all treatments and procedures in accordance with established evidence-based policies, procedures, and patient care standards. The RN collaborates with physicians and all healthcare team members in executing these tasks.
- Documentation: The RN meticulously documents relevant patient information in a timely manner, utilizing the nursing process on all required forms to ensure effective communication and collection of patient data, ultimately supporting excellent patient outcomes.
- Medication Management: The RN oversees all aspects of the medication process, including assessment, administration, monitoring, and evaluation of patient responses. The RN educates patients and their support systems on the safe use of medications and coordinates referrals to pharmacy services as necessary.
- Quality Improvement Participation: The RN actively engages in the Quality Performance and Improvement (QPI) process to enhance patient outcomes and refine processes. This involvement includes developing QPI plans by identifying actual or potential issues, collecting data, analyzing results, implementing corrective actions, and sustaining improvements. The RN contributes to maintaining and enhancing the Culture of Patient Safety.
- Delegation and Mentorship: The RN accurately delegates aspects of patient care to LPNs, CNAs, and other ancillary personnel based on their scope of practice, job descriptions, individual competencies, and patient needs. The RN supervises the provision of care by clearly communicating tasks and providing direction and support. The RN also orients, mentors, and fosters the professional development of colleagues.
Qualifications:
- Graduate of an accredited nursing program, currently licensed as a Registered Nurse in New York State.
Experience:
- Fundamental knowledge of nursing theory and practice.
- Familiarity with relevant technology and practice standards for the area of responsibility.
- Demonstrates sound clinical skills and positive communication abilities.
Licensure:
- New York State Registered Nurse license is required.
