About This Opportunity

The Assertive Community Treatment Team Nurse Practitioner plays a vital role in enhancing the quality of life for individuals with mental disorders. As a key member of our team, you will evaluate, diagnose, and prescribe medications under the direction of a qualified healthcare provider.

Key Responsibilities
  1. Assess individuals for mental health disorders and make an appropriate DSM diagnosis, ensuring services are delivered in a medically appropriate manner.
  2. Review past treatments, evaluations, and relevant information to develop an appropriate diagnosis and person-centered plans.
  3. Prescribe medication when appropriate, under the supervision of a physician or according to established policies and protocols.
  4. Request diagnostic lab work when necessary.
  5. Maintain accurate documentation and records, including the use of an Electronic Health Records system.
  6. Provide education to individuals about their diagnosis and recommend appropriate treatment, involving family members and other supports when necessary.
  7. Collaborate with other providers to discuss treatment plans, progress, and medical aspects of care.
  8. Participate in staffing of complex or high-risk individuals and attend required meetings and training.
  9. Demonstrate knowledge of agency policies and procedures, as well as service definitions related to specific program areas.
Requirements
  • Masters in Nursing (Required)
  • North Carolina Driver License (Valid)
  • PMHNP - NC Board of Nursing Certification
  • Nursing Experience
