Assertive Community Treatment Team Nurse Practitioner

The Assertive Community Treatment Team Nurse Practitioner plays a vital role in enhancing the quality of life for individuals with mental disorders. As a key member of our team, you will evaluate, diagnose, and prescribe medications under the direction of a qualified healthcare provider.
Key Responsibilities- Assess individuals for mental health disorders and make an appropriate DSM diagnosis, ensuring services are delivered in a medically appropriate manner.
- Review past treatments, evaluations, and relevant information to develop an appropriate diagnosis and person-centered plans.
- Prescribe medication when appropriate, under the supervision of a physician or according to established policies and protocols.
- Request diagnostic lab work when necessary.
- Maintain accurate documentation and records, including the use of an Electronic Health Records system.
- Provide education to individuals about their diagnosis and recommend appropriate treatment, involving family members and other supports when necessary.
- Collaborate with other providers to discuss treatment plans, progress, and medical aspects of care.
- Participate in staffing of complex or high-risk individuals and attend required meetings and training.
- Demonstrate knowledge of agency policies and procedures, as well as service definitions related to specific program areas.
- Masters in Nursing (Required)
- North Carolina Driver License (Valid)
- PMHNP - NC Board of Nursing Certification
- Nursing Experience
