Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Denali Center, a 90-bed comprehensive short and long-term care facility. As an LPN, you will provide high-quality nursing care to our adult patient population, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Observe, contribute to, and implement patient care plans, ensuring the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of patients are met.
  • Provide care based on delegated interventions identified in care plans and medical orders, within the scope of practice.
  • Document patient progress, observations, and interventions in the patient record, ensuring accurate and complete communication with other caregivers.
  • Collaborate with the interdisciplinary team to coordinate patient care, including admission, discharge, and transfer processes.
Requirements
  • Current, valid LPN or LVN license in the state of practice.
  • Completion of a state-approved IV therapy program may be required in certain facilities and/or departments.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Excellent communication and documentation skills.
