Key Responsibilities- Observe, contribute to, and implement patient care plans, ensuring the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of patients are met.
- Provide care based on delegated interventions identified in care plans and medical orders, within the scope of practice.
- Document patient progress, observations, and interventions in the patient record, ensuring accurate and complete communication with other caregivers.
- Collaborate with the interdisciplinary team to coordinate patient care, including admission, discharge, and transfer processes.
- Current, valid LPN or LVN license in the state of practice.
- Completion of a state-approved IV therapy program may be required in certain facilities and/or departments.
- Ability to work collaboratively as part of an interdisciplinary team.
- Excellent communication and documentation skills.
