Palliative Care Nurse

We are seeking a skilled and compassionate Registered Nurse to join our Palliative Care team at Illumia. As a Palliative Care RN, you will play a vital role in providing high-quality end-of-life care to patients and their families.
Key Responsibilities- Conduct in-person and telephonic assessments to identify patient support needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to provide education and support to patients and families.
- Coordinate care with palliative team members to ensure seamless delivery of services.
- Document timely clinical records and communicate findings to the PCP or AIM Palliative NP.
- Support hospice referral as appropriate and facilitate in-person visits to patients in their homes or facilities.
- 2 years of prior skilled Registered Nurse experience.
- Demonstrated ability to provide quality patient care.
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
