**Key Responsibilities:**

* Conduct thorough evaluations and assessments to identify patient needs and develop individualized therapy plans
* Provide direct patient care, including therapy sessions and documentation
* Collaborate with other healthcare professionals to ensure seamless care coordination
* Participate in ongoing education and training to stay up-to-date on the latest best practices and technologies
* Contribute to the development and implementation of departmental programs and initiatives

**Requirements:**

* Master's degree in Physical Therapy or Occupational Therapy
* 4 years of experience as a PT or OT
* Current state licensure as a Physical Therapist or Occupational Therapist
* Basic Life Support certification
* Ability to lift up to 50 pounds occasionally, and/or up to 20 pounds frequently, and/or up to 10 pounds constantly
