RN Discharge Planner

Centurion Health is seeking a skilled RN Discharge Planner to join our team at Maryland Correctional Institution in Jessup, Maryland. As a key member of our multidisciplinary team, you will be responsible for completing discharge planning for patients referred to our team based on the determination that community resources will be required at the time of release.

• Develop and implement timely and appropriate discharge plans for patients
• Act as a liaison between correctional healthcare teams and community agencies
• Collaborate and coordinate with all parties regarding patient discharge planning
• Ensure seamless transitions of care for patients

• Active RN license to practice in Maryland
• Minimum of one year of experience as a Discharge Planner for a correctional or acute care facility
• Experience working in corrections preferred
• Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR)/ BLS
• Ability to obtain a security clearance, to include drug screen and criminal background check
