Care Coordinator for Behavioral Health Services

We are seeking a compassionate and skilled Registered Nurse to join our team at Haven Behavioral Healthcare. As a Care Coordinator, you will be responsible for conducting patient assessments, providing nursing interventions, and ensuring the delivery of high-quality care in a supportive team environment.

Key Responsibilities- Conduct thorough patient assessments and develop individualized care plans.
- Provide direct nursing care, including administering medications and performing treatments as assigned.
- Maintain a safe and efficient working environment by adhering to facility policies and procedures.
- Communicate effectively with the treatment team to ensure seamless care coordination.
- Supervise and mentor Behavioral Health Technicians as needed.
To be successful in this role, you should possess:
- A graduate degree from an accredited RN program.
- Current licensure as a Registered Nurse in the state of practice.
- Experience in behavioral health or a related field (desired).

- A salary range of $65,000 - $85,000 per year.
