We are seeking a qualified Mental Health Professional to join our Community Support Team in the Hickory area. This role involves providing intensive case management and support services to adults with mental health needs, empowering them to live fulfilling lives within their communities.

Job Responsibilities:
  • Develop personalized relapse prevention and disease management strategies for individuals.
  • Facilitate access to appropriate psychological and psychiatric evaluations.
  • Conduct comprehensive functional needs assessments to determine service requirements and anticipated outcomes.
  • Assist with housing searches, including engaging landlords and writing reasonable accommodation letters.
  • Connect individuals with financial and in-kind resources to establish and maintain their households.
  • Provide ongoing support and guidance to promote emotional well-being, social integration, and independent living skills.
Qualifications:
  • Bachelor's degree in Human Services, Social Work, Special Education, or a related field.
  • One year of professional experience working with adults experiencing mental health challenges (AP) or two years (QP).
  • Strong computer skills, including proficiency in Microsoft Office Suite.
  • Excellent communication, interpersonal, and problem-solving abilities.
  • Commitment to providing person-centered care and empowering individuals.
