CenterWell Home Health is seeking a skilled Speech Language Pathologist to join our team. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.
Key Responsibilities:- Evaluate and treat patients with speech, language, hearing, and swallowing disorders
- Develop and implement individualized treatment plans to meet patient needs
- Collaborate with healthcare teams to ensure comprehensive care
- Provide education and training to patients and their families
- Document patient progress and maintain accurate records
- Master's degree in Speech Language Pathology or related field
- Current certification in Speech Language Pathology or Audiology
- Minimum 6 months of experience as a Speech Therapist
- Home Health experience preferred
- Current CPR certification
- Good organizational and communication skills
