Job Summary

We are seeking a highly skilled Behavioral Health Specialist to join our team at Carle Health. As a key member of our community-based mental health services, you will provide critical support to individuals experiencing serious mental illness.

Key Responsibilities
  • Provide community-based behavioral health services, including assessment, treatment planning, and case management.
  • Offer transportation to clients as clinically appropriate.
  • Complete assessments and treatment plans with clients.
  • Request consultation when indicated and in a timely manner.
  • Provide on-call interventions for ACT clients, covering after-hours, overnights, and weekends as assigned.
  • Conduct crisis assessments and interventions, complete safety plans with clients, and follow up after crisis situations.
  • Coordinate with Emergency Response Services and other crisis teams when clinically appropriate.
  • Provide individual and group ACT services, including monitoring daily living activities, assisting with employment and education, supporting symptom management, and advocating for clients' rights and preferences.
  • Maintain accurate records in accordance with TrilliumPlace policies and state and federal oversight agencies.
  • Act as a liaison and resource between the community and clients' needs, coordinating treatment goals with community agencies, primary care physicians, the court system, and managed-care entities.
Requirements
  • Participate in in-service and competency training.
  • Participate in interdisciplinary team meetings.
  • Prepare special reports as needed on activities and work group activities.
  • Meet clinical productivity standards.
  • Meet compliance standards for all documentation and clinical charts.
