Join Our Team as a Physical Therapist - Home Health

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in Seabrook, Hampton, and Exeter, NH. As a Physical Therapist - Home Health, you will provide rehabilitative care and skilled nursing to clients in their homes, promoting their independence and well-being.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Implement manual therapeutic exercises, gait training, and other interventions to improve clients' physical function.
  • Collaborate with nursing staff to revise care plans and ensure seamless care delivery.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
Qualifications:
  • A current New Hampshire Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and experience with electronic medical records (EMR) preferred.
