Job Summary

We are seeking a skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Optum Home & Community Care. As a key member of our care coordination team, you will play a vital role in delivering high-quality patient care and improving health outcomes.

Key Responsibilities
  • Conduct comprehensive assessments and document findings in a concise and compliant manner.
  • Ensure accurate and timely documentation of diagnoses, medications, and quality elements.
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Participate in quality improvement initiatives and contribute to the development of best practices.
  • Travel between care sites and attend mandatory educational and computer/virtual training requirements.
Requirements
  • Current licensure as a NP or PA in the state of practice.
  • Board certification through a national board (e.g., AANP, NCCPA).
  • Active DEA licensure/prescriptive authority or ability to obtain post-hire.
  • Access to reliable transportation and ability to navigate various dwelling conditions.
  • Availability to work 24 hours per month, with flexibility to work off-hours (after 5 pm, weekends, and/or holidays).
Preferred Qualifications
  • 1+ years of hands-on post-grad experience in Long Term Care.
  • Understanding of Geriatrics and Chronic Illness.
  • Proficient computer skills, including electronic medical records.
  • Ability to develop and maintain positive customer relationships.
