  • Proactively support our highest-need patients by providing comprehensive assessments, telephonic check-ins, and in-person visits.

  • Maintain extensive knowledge of community resources and make referrals to help ensure patients' healthcare and psychosocial needs are met.
About You:
  • You have a Master of Social Work degree from a CSWE-accredited school of social work.
  • You are minimally licensed in the state where OSH Center is located.
  • You possess fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary).
  • You are proficient in using Microsoft, GSuite, and electronic medical records.
  • You have US work authorization.
