We are seeking a highly skilled I/DD Population Specialist to join our team at Equiliem Healthcare.
Job SummaryThe successful candidate will be responsible for conducting assessments using the person-centered approach, working with individuals and their families to provide support and services.
Responsibilities- Schedule and coordinate assessments with individuals, families, and providers in the home and community.
- Conduct in-person interviews and gather information from multiple sources to complete assessments.
- Work with Operations Support to arrange interpreter services as needed.
- Complete all assessments using provided software and code accurately based on training.
- Document case activity and meet productivity standards.
- 1 year of experience working with individuals with intellectual and/or developmental disabilities.
- Bachelor's degree in an approved human service discipline.
- $65,000 - $80,000 per year (estimated salary range).
