Responsibilities
- Provide direct patient care, including assessments, medication administration, and documentation.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Conduct thorough assessments of residents' physical and emotional needs.
- Administer medications and treatments as prescribed by physicians.
- Document patient progress and communicate effectively with the healthcare team.
- Participate in quality improvement initiatives and contribute to a positive and supportive work environment.
Requirements
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of Wisconsin.
- Active CPR certification.
- Experience working in a geriatric setting, preferably in a long-term care or assisted living environment.
- Knowledge of state and federal regulations governing long-term care.
- Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
- Excellent communication and interpersonal skills.
