Position Overview

Core Objective:

The individual in this role is an essential member of the Patient-Centered Medical Home team, contributing significantly to the delivery of patient-focused healthcare. This position involves assisting healthcare providers and the team in the comprehensive coordination, preparation, examination, and treatment of patients. The individual will fulfill the essential competency requirements for a clinical assistant.

Key Responsibilities:

  • Prepares for patient visits by reviewing medical records
  • Notifies patients of their appointments as needed
  • Confirms completion of prior medical orders
  • Ensures all necessary documentation is available
  • Identifies care gaps as necessary

Patient Check-In Duties:

  • Accurately records vital signs and other health metrics
  • Updates information related to healthcare quality measures
  • Conducts necessary clinical procedures according to established protocols

Patient Check-Out Duties:

  • Efficiently executes clinical procedures and orders as directed by the healthcare provider
  • Administers prescribed medications
  • Assists with basic wound care as required
  • Refers patients to specialists when necessary
  • Completes laboratory requisitions accurately

Administrative Responsibilities:

  • Prepares examination rooms for patient visits
  • Maintains inventory of medical supplies
  • Completes necessary documentation
  • Follows up on missed appointments and reschedules as needed
  • Sanitizes, labels, and organizes medical instruments as required
  • Handles communication with patients, staff, and community agencies
  • Contacts patients regarding laboratory results and other reports
  • Schedules appointments as necessary

Operational Duties:

  • Utilizes basic medical equipment effectively
  • Documents patient interactions consistently in the electronic medical record (EMR)
  • Collaborates with healthcare providers and team members to enhance patient care

Consistent and reliable on-site attendance is essential.

Additional Responsibilities:

  • Performs other related tasks as assigned
  • Participates in team meetings, educational programs, and required training sessions
  • Exhibits excellent customer service to patients and providers, promoting a positive image of the organization
  • Provides constructive feedback for quality improvement initiatives
  • Arrives at work punctually and informs supervisors of any delays or absences
  • Answers phone calls promptly and takes accurate messages
  • Welcomes feedback during evaluations and demonstrates a commitment to improving job performance
  • Maintains and enhances personal professional skills
  • Adheres to safety and security protocols, reporting any potentially hazardous conditions

Qualifications:

Skills and Knowledge:

  • Ability to perform basic arithmetic operations and understand measurements
  • Ability to compute rates, ratios, and percentages, and interpret graphical data
  • Ability to follow written, oral, or diagrammatic instructions
  • Ability to address problems involving concrete variables in standardized situations

Education:

  • High School Diploma or GED is required
  • Certification as a Medical Assistant is required
  • Completion of a one-year certificate program from a college or technical school, or an equivalent combination of education and experience is required
  • Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification is required

Experience:

  • Prior experience with basic computer systems is required
  • Familiarity with basic medical terminology is required
  • Bilingual candidates are preferred
