Optum House Calls Nurse Practitioner Opportunity

We are seeking a skilled Nurse Practitioner to join our House Calls team at Optum. As a Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities:

  • Conduct comprehensive in-home assessments, including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as needed.
  • Identify diagnoses and develop care management plans to address patient needs.
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination.
  • Provide education to patients on disease management, medication adherence, and healthy lifestyle choices.

Requirements:

  • Active NP license and ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care.
  • Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting/standing/kneeling.
  • Reliable transportation for home visits.

Preferred Qualifications:

  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
