We are seeking a highly skilled and compassionate Registered Nurse to join our team at Commonwealth Home Health, a part of LHC Group. As a Registered Nurse, you will have the opportunity to provide high-quality patient care in a home health setting, working closely with our interdisciplinary team to deliver comprehensive care to our patients.
Responsibilities- Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop individualized plans of care.
- Coordinate total plan of care with other healthcare professionals, including physicians, therapists, and social workers, to ensure continuity of patient care.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
- Monitor patient progress, adjust plans of care as needed, and communicate effectively with patients, families, and healthcare team members.
- Ensure compliance with regulatory requirements, agency policies, and payer source guidelines.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
