Home Health Occupational Therapist

FeldCare Connects, a leading referral network, is seeking an experienced Occupational Therapist to deliver high-quality care in the Port Hueneme area.
The ideal candidate will possess strong assessment and evaluation skills, with the ability to create effective treatment plans that improve patient functionality and wellbeing.
Responsibilities:
- Conduct comprehensive assessments and evaluations of patient care needs, including functional status, activities of daily living, and fine motor coordination.
- Determine treatment plans based on patient needs and goals, and implement interventions aimed at improving patient wellbeing.
- Evaluate patient progress and make adjustments to treatment plans as needed.
- Provide education and training to patients, families, caregivers, and healthcare teams on occupational therapy techniques and strategies.
Qualifications:
- Occupational Therapist license and registration by the state.
- Completion of an accredited Occupational Therapist program.
- Ability to work with patients of all ages, with a focus on adults.
- Bilingual skills are a plus.
