Responsibilities:- Identify and address the ongoing needs and services of residents to promote their overall well-being.
- Ensure proper follow-through and participate in evaluations and documentation for residents with changes in healthcare services and status.
- Assist the Resident Care Director in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Conduct monthly wellness visits for all residents and monitor and follow up with residents who have experienced falls, accidents, or incidents.
- Document all pertinent information in the resident wellness file and maintain communication with residents' families and responsible parties.
- Collaborate with the Resident Care Director and community team to promote infection control standards and provide clinical support and assistance as needed.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year of experience in home health, assisted living, hospital, or long-term care environment.
- Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
