Job Title: Nurse Practitioner - Home Health Care

Premium Choice Physicians is seeking a skilled Nurse Practitioner to join our team in providing high-quality home health care services to patients of all ages.

Key Responsibilities:
  • Conduct thorough in-home assessments and evaluations to certify home health care, including vital sign checks, medical history reviews, and medication management.
  • Obtain patient consents and submit them to the intake team within 24 hours.
  • Complete electronic medical record (EMR) documentation within 24 hours of each patient visit.
  • Develop and implement personalized treatment plans, taking into account the patient's current condition and medical history.
  • Facilitate referrals to other healthcare professionals and medical facilities as needed.
  • Prescribe medications and order medical tests as appropriate.
  • Provide patient education and support to promote healthy outcomes.
  • Practice advanced nursing skills in a consistent and prudent manner.
  • Participate in bi-annual ride-alongs with the Medical Director for practitioner evaluation and process improvement.
  • Communicate effectively with the scheduling office via email, phone, or documentation.
  • Discuss any issues or concerns with the Medical Director.
  • Maintain a professional appearance, adhering to dress code standards.
  • Demonstrate promptness, courtesy, and a professional demeanor at all times.
  • Report patient concerns immediately, prioritizing patient safety and well-being.
  • Attend regular Nurse Practitioner provider meetings.
Required Qualifications:
  • Current licensure with the Texas Board of Nursing.
  • Basic Life Support certification.
  • Ability to obtain DEA licensure.
  • Experience with electronic medical records (EMRs).
  • Certification from the ANCC, AANP, or AACN as an Adult, Geriatric, Family, or Acute Care Nurse Practitioner.
  • At least 1 year of clinical experience in the highest level of education or 3+ years of experience as a Registered Nurse in Acute Care, ED, Home Health, or Hospice.
  • Excellent communication, collaboration, problem-solving, and organizational skills.
  • Ability to build rapport with patients and their families.
  • Excellent written and verbal communication skills.
  • Ability to spend at least 1 hour with patients in their homes, which may involve remote areas.
  • Compliance with HIPAA regulations and maintenance of patient confidentiality.
  • Ability to lift 20-pound bags and navigate various dwelling conditions.
  • Valid driver's license with a good driving record; transportation coordination will be provided after 90 days.
Preferred Qualifications:
  • ACLS certification.
  • Experience in Gerontology, Cardiology, Internal Medicine, and/or Endocrinology.
  • Home care or home visit experience.
  • Excellent administrative and organizational skills, with the ability to effectively communicate with patients and their families.
  • Computer literacy and proficiency in using GPS navigation systems.
