Position Summary
We are seeking a skilled Registered Nurse to assess, plan, implement, and evaluate nursing care for patients with chronic organ diseases and potential transplant candidates. The RN will coordinate care among the transplant team to ensure optimal patient outcomes.
Key Responsibilities
In this role, you will be responsible for:
Pre-Transplant Phase:
- Conducting necessary evaluations to determine patient eligibility.
- Providing education to patients and their families.
- Participating in patient selection and evaluation processes.
- Monitoring patient status throughout the work-up and listing phases.
- Preparing patients for transplantation surgery.
- Managing data collection for regulatory compliance.
Transplant/Inpatient Phase:
- Leading educational efforts for patients and families.
- Participating in clinical rounds to monitor patient quality and safety.
- Maintaining communication with the multidisciplinary team.
- Serving as a resource for transplantation information.
- Adjusting care plans based on ongoing assessments.
- Preparing patients for discharge and follow-up care.
- Ensuring continuity of care through effective communication.
Post-Transplant Phase:
- Monitoring diagnostic studies and health status.
- Coordinating patient concerns with the transplant team.
- Continuing education for patients and families.
- Providing care under the direction of the transplant physician.
- Managing data collection for regulatory compliance.
Qualifications
To be considered for this position, you should have:
- A degree from an accredited nursing program (diploma, associate, or bachelor’s).
- A minimum of one year of RN experience, meeting all RN II requirements.
- Basic Cardiac Life Support certification within 30 days of hire.
