As a Clinical Pharmacist with Grane Home Hospice Care, Inc, you will play a vital role in ensuring the safe and effective use of medications in our healthcare settings. Your expertise will be instrumental in evaluating physician medication orders, making recommendations for therapy changes, and collaborating with the healthcare team to provide clinical and technical support.
Responsibilities:
Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
Make recommendations for therapy changes to the physician and facility staff as appropriate.
Maintain patient/resident database to include appropriate current and past medical history, clinical and laboratory data, and concurrent medication.
Ensure that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
Collaborate with the healthcare team to provide clinical and technical support as needed.
Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and policy.
Inform the Pharmacy Manager of medication-related errors.
Qualifications:
Bachelor's Degree in Pharmacy; Pharm D preferred.
One year experience in long-term care.
Licenses/Certifications: As required by government regulations and business needs.
Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
