Medical Professional for Home Risk Assessments

We are seeking a skilled medical professional to join our team at Matrix Medical Network as a Nurse Practitioner or Physician Assistant. This is a PRN/Part Time position, and you will have the opportunity to work in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
Your primary responsibility will be to conduct comprehensive health assessments, review medical history, diagnose and treat patients, and provide education on health and wellness. You will also be responsible for collaborating with physicians, case managers, social workers, and other healthcare professionals to ensure that patients receive the best possible care.

Responsibilities- Conduct adult/geriatric assessments, review medical history, diagnose and treat patients, and provide education on health and wellness
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to ensure that patients receive the best possible care
- Work in a variety of venues, including home visits, skilled facility visits, and telehealth visits
- Master's degree in nursing or a related field, OR commensurate experience and satisfactory completion of NP licensure
- Current RN and NP or PA licensure in the state of practice, with prescription authority or the ability to obtain prescriptive authority
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant, with strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills, with a positive and outgoing personality
Earn up to $11,500 per month (based on an average 100 completed visits per month, and pay of $115 per completed assessment). Compensation range is based on availability and assessment completion.
